Accident Investigation

Every company experience incidents that may threaten the safety for people, environment or assets. Some companies even experience tragic accidents. Accidents and incidents are important information sources for improving the company's safety and risk handeling systems. The purpose of an investigation is not to find a guilty person to blame, but rather finding the root causes and weaknesses in the organisation which can be improved in order to avoid new accidents.

Scandpower performs a range of services within the field of Accident Investigation:

• Participation in the company's internal investigations, either as a investigation leader or as a member of the investigation team
• Perform third party inquiries in those cases where the companies find it difficult to investigate themselves or where they are asked to have an independent investigation
• Accident Investigation courses. Scandpower base their courses on the MTO methodology (Man, Technology and Organization). The courses are meant for both investigation leaders and members of the investigation team. The courses will give the participants the opportunity to learn how to: 
  -  find the course of events 
  -  find the direct causes and root causes, 
  -  find the barriers (broken/worked) 
  -  establish the connection between causes and measures.
• Develop the sequence of events and MTO diagrams. If the companies perform the accident investigation themselves, Scandpower can help to make the MTO diagram
• Mapping/trending of events over a period of time
• Follow-up after serious incidents, evaluation of the process of improvements and the effect of the implemented measures.
